Commonwealth of Pennsylvania is now requiring all requests for birth and death records to be done on new forms that have the requester sign a release that they are who they say they are. The old forms are no longer acceptable and will be returned.

Also, ALL requests for marriage and divorce records should be directed to the County Courthouse in which the marriage license or divorce decree was issued.
