I the Orphans' Court of Berks County.
To the Honorable H Willis Bland,
President Judge of the Orphans' Court of County aforesaid:
The Petition of Annie L Smeck respectfully represents:
That she is a minor child over the age of fourteen years; that she resides in Reading, Berks County aforesaid; she was born on the 19th day of November A.D. 1882; that she is the child of Albert & Emma Smeck, deceased; that she has no guardian to take care of her estate; no former guardian having been appointed she therefore respectively prays the Court to appoint Henry T Laurish of Reading, Pa., guardian for the purpose aforesaid. And she will ever pray, &c.

Personally appeared before me, the understanding authority Annie L Smeck who being duly sworn according to law, disposes and says, that the facts set forth in the above petition are true to the best of her knowledge and belief; ant that the said Henry R Laurish is neither executor nor administrator of any estate in which the said minor is interested.

And now to wit: Dec 31 1901, the within petition having been presented and read, the Court admit the petitioner to make choice of a guardian: and the minor having made choice of Henry R Laurish the Court appoints the said Henry R Laurish to be guardian of said minor.
